Application Description

The form is designed with anonymity in mind — personal information fields are optional allowing submitters to report concerns without identifying themselves if they choose to do so. Submitters can specify the agency or individual the complaint is directed against, provide their own contact information if desired, and upload any supporting documentation they may have to strengthen their submission. All complaints are reviewed and managed from a centralized administration dashboard with full status tracking, approval workflow, and export capabilities, ensuring every concern is handled with the appropriate level of attention and confidentiality.
